District Nurses (Tel: 0191 5026426)
The District Nurses visit housebound patients or patients who are temporarily unable to get to the surgery. Our aim is to provide a high standard of nursing care in the home and improve a patient’s quality of life. District nursing intervention covers a broad spectrum of care from basic wound care to terminal nursing care. The assessment involves not only physical but social, psychological and spiritual aspects of care to meet the needs of patients and their families. In order to do this we utilize and co-ordinate the services of other health professionals to ensure patients and families needs are considered.
We perform a wide range of nursing and medical procedures including:
- Total assessment
- Identify patient needs
- Plan and educate patient’s care
- Health promotion
- Education and advice
- Support and counselling
